Abstract

A 55-years-old male, presented with complaint of gradual diminution of vision in Right eye since 6 months. Patient underwent Right eyes cataract surgery one year back.On presentation vision in Right eye (BCVA) was 3/60, in left eye was 6/6.Dilated examination revealed significant postoperative opacification of intraocular lens (IOL) along with a localised wide splits on the anterior surface of the lens,area of delamination was seen on the anterior surface of the Intraocular lens.

These IOL opacification are uncommon,also termed as”Tertiary Cataract” which can be seen in hydrophilic/hydrophobic acrylic IOLs & Silicon IOLs.It can be seen as glistening, diffuse granular opacity,and crust like opacification, usually caused by precipitation of crystalline deposits on the IOL’s surface,which can be related to the viscoelastic substance or the balanced saline solution used in the surgery.If visually significant,IOL Exchange surgery can be performed,which is associated with good visual outcome
