Abstract

A Systemically healthy 91-year-old pseudo phakic male presented with RE- V/A:6/6, LE-painless dimness of vision-V/A: PL only, with 270-degree conjunctival mass encroaching over cornea, which was clinically aggressive for ocular surface squamous neoplasia (OSSN) and was confirmed by histopathology without lymphovascular embolization and perineural invasion, in absence of risk factors like UV radiation, Xeroderma pigmentosa and HIV. MRI- orbit suggest possibility of infective collection and MRI brain showed complete thrombotic occlusion of right ICA for which started on anti-coagulant. Patient underwent surgical excision of extensively invasive OSSN along with excision of margins and adjuvant cryotherapy and amniotic membrane grafting. He showed improved vision in LE-CF 2 MT. Salvaged eye had event free follow up. Isolated invasive OSSN is rare with few case report. Surgical excision for extensively invasive OSSN offered an alternative to enucleation in selected case of nonagenarian.
