Abstract
2 year old girl presented with rapid onset proptosis and total limitation in ocular motility of right eye since 3 days, associated with right cheek swelling. Pupillary reaction was sluggish with faint retinal glow. Orbital cellulitis was suspected due to associated URTI. Lack of improvement after systemic antibiotics and normal counts prompted CT scan, which revealed well defined soft tissue lesion filling the intra and extraconal compartment with associated bony erosion, sinus involvement and intracranial extension. Clinical suspicion of neoplasia was confirmed by incisional biopsy. Histopathology showed presence of elongated spindle cells suggestive of alveolar rhabdomyosarcoma. Due to extensive intracranial involvement, the patient was advised radio and chemotherapy prior to surgical intervention.
