Abstract
A 50 year old gentleman came to us with gradual diminution of vision in right eye since 4 years following cataract surgery .At presentation vision in right was counting finger 2 metre with + 4.00 spherical addition. On examination Anterior chamber was quiet, IOL was in place. Intraocular pressure( IOP) recorded as 6 mm of Hg as compare to left eye which was 18 mm of Hg. Fundus examination showed Attached retina, multiple choroidal folds, disc edema and peripheral shallow choroidal detachment. UBM showed cyclodilysis cleft, OCT macula showed choroidal detachment. Patient was started on eye drop atropine sulphate 1% 3 times per day, and being followed regularly. Patient vision improved to 6/36 after 3 months of treatment and IOP was recorded as 12 mm of Hg. Hypotony with cyclodialysis cleft most of the time requires surgical treatment . Our case was managed medically with Atropine sulphate eye drops. Medical management is an effective way of treating hypotony.
