Abstract
A 42 years old male presented with the chief complaints of pain, redness, diminution of vision in the left eye for 10 days, with a history of splashing muddy water from paddy field 13 days ago. His VA was hand movement in L/E. On examination, a stain positive centrally located, well-circumscribed corneal ulcer of size 7mm*8mm in dimensions was found. He was started on E/D Natamycin, E/D Moxifloxacin & E/D Atropine. On day 6, a hypopyon of height 2mm was found. Culture report revealed hyaline fungi of species Chrysosporuim. His viral PCR also came to be HSV 1 positive. E/D Natamycin was stopped and E/D Voriconazole was started along with E/D Azithromycin, E/D Atropine and E/D Dorzolamide+Timolol. But even after maximum medical management, having found no improvement, patient was taken up for Penetrating Keratoplasty after a week. His VA improved drastically to 6/60 at post-operative day 3 and after a month, his best corrected visual acuity was found to be 6/9 in left eye.
