Abstract
Monocular elevation deficiency is characterized by inability to elevate one eye above the horizontal plane.We present a case of 8 years male presented to OPD with complaint of right sided drooping of upper lid since birth. General and systemic examination was unremarkable. Visual acuity was 6/6 in each eye (Log MAR= 0.00). Diffuse light examination shows chin lift and right eyebrow at higher level. Pupil was of normal size and reacting to light. Anterior and posterior segment examination was normal. Ptosis evaluation of RE shows marginal reflex distance 1= -2mm, vertical fissure height= 7mm, LPS action= 2mm, absent lid crease, poor bell’s phenomenon, restricted ocular movements on dextro and levo elevation and 8mm of schirmer strip wetting at 5 minutes. No mechanical restriction felt in forced duction test and force generating capacity in upgaze was less as compared to left eye. Patient was planned to undergo frontalis sling surgery (RE) under LA.
