Abstract

A 28 year old man presented to the OPD with complaints of sudden, painless loss of vision in both eyes since 1 week. BCVA (RE) is 6/36 and BCVA (LE) is 6/60. Anterior segment of BE was normal except for bilateral pallor of tarsal conjunctiva. Fundus examination of the right eye revealed a typical boat-shaped subhyaloid hemorrhage inferior to the macula, multiple superficial and deep retinal hemorrhages along with Roth spots. Fundus of the left eye showed multiple retinal hemorrhages involving the macula. Blood investigations showed Hb-5.6g/dl and B12-100pg/dl.

Treatment of anemia slowly led to the resolution of the hemorrhages ultimately improving the visual acuity.

This case highlights the importance of eye as a window to the body. Ophthalmological findings can help detect certain systemic conditions which could have been missed otherwise. Timely diagnosis and treatment of those systemic conditions can lead to resolution of the ophthalmological signs thus improving visual prognosis.
