Abstract

A 42 year old male patient presented to our department with complaints of gradual dimunition of vision along with inability to move either eye in any direction. On external examination patient had severe blepharoptosis in both eyes . His right eye was operated for ptosis by a different surgeon earlier. He had typical features of myotonic dystrophy like hatchet face and swan neck. All extraocular muscles were restrictred completely in both eyes. Forced duction test was performed which confirmed bilateral restrictive ophthalmoplegia. His vision was 6/12 in both eyes with early nuclear sclerosis. Slit lamp examination showed mild exposure keratopathy in the right eye. Anterior segment was normal. His IOP was 18 mm of Hg in both eye. Fundus examination showed synchisis scintillans in the right eye while left eye was normal. Unfortunately genetic studies could not be done for this patient.
