Abstract
To report case of probable occult macular dystrophy(OMD) and discuss various differential diagnosis of isolated foveal photoreceptor disruption. 18-year male came with bilateral diminution of vision for 6 months. BCVA was 6/36 in both eyes. Anterior segment B/L- normal. Fundus (B/L)- normal with blunted foveal reflex. OCT- B/L foveal thinning, loss of subfoveal ellipsoid and interdigitation zone with intact RPE and no hyper-reflectivity in outer nuclear layer. OCTA–normal. Based on history and clinical findings, diagnosis of occult macular dystrophy was made.OMD can present with bilateral, isolated, foveal photoreceptor disruption. An array of differential diagnosis including vitreomacular interface disorders, photic retinopathy,macular dystrophy, drug toxicity needs to be ruled out based on detailed history, OCT and electrophysiological studies to reach diagnosis.
