Abstract
BACKGROUND: Neuromyelitis optica is an inflammatory disease, causing demyelination of central nervous system the primary targets being optic nerve and spinal cord.
CASE PRESENTATION: A 27 years old male ,known case of multiple sclerosis, who presented with 8 days history of diminution of vision in right eye. It was associated with weakness in all four limbs and difficulty evacuating bladder. Clinical examination revealed visual acuity of fc 2 mt (PR accurate) in RE and 6/6 (PR accurate) in LE, RE RAPD grade II. Rest of ocular examination was within normal limit. MRI ORBIT revealed right optic nerve tortuousity and increased bulk in retro bulbar and midorbital regions. Patient was given injection methylprednisolone for 3 days with visual recovery of 6/60 in RE. Patient was discharged on oral prednisolone and Va on day 15 was 6/18
CONCLUSION :According to ONTT trial, methylprednisolone facilitates visual recovery in the acute phase of optic neuritis.
