Abstract
Ocular syphilis has varied manifestations and may present as anterior uveitis, posterior uveitis, pan uveitis, retinitis, papillitis, and even scleritis, making it a “great masquerade”. We hereby present a case of retinal vasculitis with retinitis in a young lady diagnosed as ocular syphilis.
A 40-year-old female presented with painless DOV OS for 3 days. Visual acuity(VA) at presentation as 6/12, 2/60 in OD, OS respectively. On anterior segment examination,OD showed healed interstitial keratitis & ghost vessels. OD fundus was WNL whereas OS revealed vitritis, retinitis patch inferior to disc, phlebitis & multiple intraretinal hemorrhages. Uveitis serology was done and VDRL and TPHA were found to be positive following which patient was started with IV Ceftriaxone and Wysolone. Patient’s VA improved to 6/12 post treatment.
Our case highlights the importance of lab investigations in patients presenting with retinal vasculitis and shows early intervention can prevent severe visual loss.
