Abstract

PURPOSE: To describe a case of phacocele following a trivial trauma in a known case of rheumatoid arthritis with scleromalacia perforans

METHOD: We report a case of 47 year old female who developed defective vision in the left eye following a slip and fall while walking. She was a known case of rheumatoid arthritis with scleromalacia perforans. She developed well circumscribed swelling in the superonasal quadrant. Visual acuity was HM+. In the CT head a dislocated lens was seen anterosuperiorly. After undergoing wound exploration , lens extraction , scleral repair patient was left aphakic. Globe integrity was maintained. Post operatively barrage laser was done superiorly and on aphakic correction patient got a vision of 6/60 in the left eye.

RESULT: Successful and timely intervention can prevent development of disabling sequelae

CONCLUSION : Proper care in patients with scleral thinning is necessary as trivial trauma can lead to severe problems
