Abstract
18yr/M complaining sudden diminution of vision,pain&redness in both eyes since 2 days with neck stiffness.No history of ocular trauma/surgery/spectacles use. No significant systemic/family/personal history.General&systemic examination was WNL.Ophthalmic examination:visual acuity RE1/60,LE 2/60 with BE circumcorneal congestion&3+ cells in AC.BE IOP normal.Fundus examination BE:no significant vitreous haze, hyperemic disc+, subretinal fluid pouches involving fovea.BE OCT:RPE undulations with multiple neurosensory RD.BE FFA:pin point leaks in early phase followed by diffuse pooling of dye.Blood investigations:raised ESR.Started on injection Methyl Prednisolone 1g in 400ml NS iv slowly over 4hrs for 3 days with strict B&BSL monitoring f/b oral T Prednisolone 40mg OD tapered over 5 days with T Azathioprine 50mg BD with calcium supplements.The visual acuity gradually improved to 6/9 in both eyes on Follow up visit(day 20 of treatment) with AC quiet, significant OCT&Fundus changes.Looks VKH!
