Abstract
A 40-year-old lady presented with diminution of vision in right eye for past 3 weeks. Old vitreous bleed was noted. Multiple small drusens were present in mid-periphery of left eye. USG revealed vitreous echoes and shallow RD temporally with subretinal echoes. Patient had normal systemic investigations. During pars plans vitrectomy, multiple mounds of subretinal and subRPE bleed were noted in temporal mid-periphery. Diagnosis of peripheral hemorrhagic chorioretinopathy was made. Intravitreal Ranibizumab and 20% SF6 gas was injected. At 1 week, BCVA was 6/12 with retina attached. At 2 week follow-up, total retinal detachment with subretinal bleed was noted. Encirclage with vitreoretinal surgery was performed. Break couldn’t be identified. Subretinal bleed was evacuated via iatrogenic retinotomy. After endolaser, silicone oil was injected. At 2week, retina was flat with BCVA of 6/60. OCT revealed residual subretinal bleed in temporal macula. Retina remained attached at 6 week follow-up.
