Abstract
Purpose: To evaluate the efficacy of Intrastromal injection of Voriconazole for treatment of resistant fungal keratitis
Methods: All the pts having microbiologically proven fungal keratitis not responding to conventional topical therapy were included. All pts were given ISVCZ around the ulcer site. Pts were followed on day 1, 2, 5,9,2wks,3wks,1month. Response to treatment in form of reduction in size and infiltration of ulcer were noted. Results: 25pts (18M/7F) with mean age 38±2.5 yrs were included. The mean time of resolution was 25±5.6days.21 pts(84%) responded to treatment & resolved while 4pts(16%) ended up with perforation. The most common organism isolated was Aspergillus followed by Fusarium & Mucor.Ulcer size and height of hypopyon were risk factors associated with treatment outcome. Conclusion: ISVCZ can be a safe and effective therapy for fungal keratitis not responding to conventional therapy. It helps in healing of ulcer, control of infection & also reduces need for TPK.
